The Assessment and Care Level dashboard provides a portfolio view of resident distribution across product types and care levels, recent assessment activity, evaluation statuses, and care level overrides. It helps clinical leadership monitor assessment compliance and understand the acuity mix across the organization.
Access the Assessment and Care Level Dashboard
The Assessment and Care Level dashboard is enabled automatically for all ALIS HQ customers.
To access it: From ALIS HQ, select Assessment and Care Level from the Dashboard menu at the top.

What This Dashboard Shows
The dashboard is organized into the following sections.
Filters
At the top of the dashboard, filter by Region, Owner, and Community.
Current Residents per Product Type and Care Level
- Current Product Type Distribution: Bar chart with the count of residents by product type (AL, MC, IL, etc.).
- Current Care Level Distribution: Pie chart showing the percentage of residents at each care level.
- Current Residents per Care Level: Table showing the exact count of residents at each care level.
Where does the data originate?
- Product Types are configured in Company Settings. Product Types are then assigned to individual residents in the Basic Information section of their profile.
- Care Levels are configured at the community-level in Resident Evaluation Tool Settings. Based on these settings, ALIS typically automatically calculates a resident's resulting Care Level during Evaluation completion. The Care Level is solidified during final review of the Service Plan.

Historical Assessments
- Assessments Over Time: Monthly stacked bar chart showing the number of evaluations completed, broken down by reason (Initial, Change of Condition, Annual).
RET Statuses
- In Progress RETs: KPI showing the count of evaluations currently in progress, with a detailed table including Community, Resident Name, Resident Stage, Reason, Evaluation Date, Completed On, and Needs Signature status.
- Expired RETs: KPI showing the count of expired evaluations, with a detailed table including Community, Resident Name, Resident Stage, Reason, Evaluation Date, Completed On, and Expiration Date.
- Service Plan Status: Table showing Completed, Draft, N/A, and Total counts by community.
- Care Plan Status: Table showing Active, Draft, N/A, and Total counts by community.
Where does the data originate?
- RET and Service Plan data comes from any resident evaluations with the "In Progress" status visible in the Evaluations & Service Plans section of the resident profile and in the Evaluation Center.
- Care Plan data comes from the "Published" status visible in the Care Plan section of the resident profile.

Override Care Level Comparison
- # of Care Level Overrides: KPI showing the count of care level overrides in the selected time range (Today, Yesterday, Last 7/30/60/90 days, plus Weeks/Months).
Where does the data originate?
- Data populates here if Care Points, Care Level, or Care Fee fields were updated when using the "Override Care Level" option upon Service Plan completion.
Access to this action is dependent on user security role permissions. For assistance with updating security roles, contact your Account Manager.
How to Use This Dashboard
- Understand your acuity mix. Use the Current Care Level Distribution to see how residents are distributed across care levels, which can inform staffing, pricing, and care planning decisions.
- Stay ahead of expirations. The Expired RETs table identifies evaluations that need re-completion. The In Progress RETs table identifies evaluations that need to be finished.
- Spot signature gaps. The Needs Signature column on the In Progress RETs table highlights evaluations that are complete but pending a signature.
- Monitor service plan and care plan completeness. The status tables surface communities where residents may be missing active plans.
- Drill down. Click on any chart, KPI, or table row to see the underlying records.
