Completing an ALIS Incident Report is a collaborative process that may involve staff and administrator participation. The goal is to ensure all required details, forms, and follow-up tasks are documented before marking the report as Complete.
Steps Overview
While workflows may vary by community or state requirements, completing an Incident Report generally includes three key steps:
- Record the initial incident details.
- Complete all required forms and tasks.
- Mark the full report as Complete.
1 - Start Initial Incident Report
Begin the report for a new incident from any of these main locations:
- Dashboard: Click the Report Incident icon.
- Incident Center: +Report New Incident button.
- Resident's profile page: +Incident button under the resident photo.
2 - Add Incident Details
Each option opens a panel on the right side of the screen to enter initial details.
Required Fields:
Incident Type: Select the type of incident. This determines which forms and tasks are required.
Resident Involved: Select the resident directly involved.
Date & Time of Incident: Auto-populates when reporting begins; update to reflect when the incident actually occurred.

Incident Summary: Enter a clear, objective description of what occurred, including severity and current status. This summary will also populate associated incident forms (if applicable).
Speech-to-Text Transcription (Optional): You can use speech-to-text to dictate the Incident Summary instead of typing by selecting the microphone icon and beginning to speak. Always review and edit the transcribed text before continuing.
After entering all required details, click Create Incident Report.
You will typically be redirected to the Incident Report page. If not, the report can be accessed later from the Resident Profile or Incident Center.
3 - Complete Forms and Tasks
If you navigate away, you can return to an incomplete report via the Incident Center and click Edit.
The Incident Report page includes three sections:
- Basic Information
- Incident Report Forms
- Incident Report Tasks
Basic Information
The following fields cannot be edited: Resident Name, Incident Type, Reported By, Created Date. If these are incorrect, the incident must be deleted and re-created.
Users with permission can edit:
- Date and Time
- Other Residents Involved
- Incident Summary
Be sure to click Save after making changes.
Incident Report Forms
This section displays all forms associated with the selected Incident Type.
- Use Fill Online (Recommended) to complete digitized forms.
- Use Choose File to upload completed paper forms.
- Use Options to print blank or pre-filled forms.
Fill Out a Form
- Click Fill Online > New Form.
- Complete required fields based on your role.
- Click Save and Close if the form is not yet complete.
- Status will then display as Fill Online in Progress.
- Click Edit to continue editing the form.
- Do not click Complete until all required sections are finished.
Complete a Form (Admin only)
- Click Complete to lock the form as a PDF with a timestamp.
- Use Fill Online > Previous Form to edit completed forms (admin only).
Incident Report Tasks
This section includes required follow-up actions and optional monitoring.
Tasks
- Tasks are assigned based on Incident Type and community settings.
- Click Mark Complete and enter a note describing the action taken.
Resident Monitoring (Optional)
- Click Monitor Resident to initiate monitoring.
- Refer to the Resident Monitoring Reference Guide for detailed instructions.
3 - Complete the full Incident Report
Once all forms and tasks are complete:
- Click Complete (administrator access typically required).
- This locks the report and updates its status to Complete in the Incident Center.
If required forms are not completed, a prompt may appear preventing completion. All incident reports remain accessible in ALIS indefinitely and can be viewed or printed at any time.
Review Completed Incident
In the Incident Center, you can:
- Review Status, Forms, and Tasks columns.
- Confirm all forms are completed (shown as links).
- Confirm all tasks are completed (shown as fraction).
If updates are needed:
- Click View → Unlock (administrator access typically required).
- Complete missing items.
- Click Complete again to relock the report.
