Recording PRN Care is ideal practice whenever you assist a resident with a task that is not currently listed on their scheduled Care Plan. Documenting these extra tasks ensures your time is accounted for and helps the clinical team identify if a resident's Care Plan needs adjustment to better meet their needs.
Access Record PRN Care
You can access the tool for recording PRN Care from two locations in ALIS:
From the Dashboard: Click the Record PRN Care icon.
In Care Tracking: Navigate to the Record Care page for any care list and click the Record PRN Care button located at the bottom left of the filters.
How to Record PRN Care
After you click the Record PRN Care button, a pop-up modal will appear.
Enter the PRN Care Item details:
Resident: Choose the name of the resident you assisted.
Care Program: (You most likely do not need to change this dropdown).

Care Item: Select the specific name of the task you performed.
Note: If you do not see the specific task listed, contact your administrator for assistance.
Verify the Recorded At and Time fields. These default to the date and exact time you clicked the button.
Time Taken (Recommended): Enter how long it took to complete the task. Recording this duration helps the team track the additional time required for care and identify emerging trends.
Select Outcome: Choose the result of the task.
Notes (Recommended): Add notes explaining why the extra care was needed. This context is critical for supervisors reviewing the resident's changing needs.
Click Done to save the record.
Recorded PRN Care
Once a PRN Care item has been recorded (regardless of whether the outcome was "Completed" or "Not Completed"), it remains viewable and editable if adjustments are needed.
View: On the Care Tracking landing page, a PRN List will automatically appear under the specific shift during which the care was recorded.
Edit: To review or edit the details of the record, locate the list and click Update Care.
Scheduled PRN Care
In some cases, PRN Care items may be scheduled in advance. This is typically used for short-term requirements, such as Alert Charting or temporary task assistance.
How do I know if a PRN Care Item has been scheduled? If a PRN task has been scheduled for a specific shift, it will appear on the Care Tracking page in the designated PRN List.
How do I record a scheduled PRN Care Item? Simply locate the item on the list and record it just as you would any other standard care task.

PRN Care Report
Use the PRN Care Report to fix scheduling errors or to review the outcomes of these short-term tasks. Print or Export to Excel this report regularly to review and spot patterns where residents require extra assistance beyond their permanent Care Plan.
To access the report, go to Reports > Care Reports > PRN Care Report.
Edit: Click the pencil icon to the left of an item to make changes to the record.
Delete: You can only delete a scheduled item if it has not yet been documented.
