Service Plan Addendums provides the ability to attach additional needs to a resident's current Service Plan without making direct changes to the original document or requiring new signatures.
This feature is behind an entitlement and requires the ALIS team to enable. To enable the entitlement, contact your Account Manager or our ALIS Customer Success Team.
Addendum Use Cases
It is important to know when to create an Addendum versus when to make direct changes to the Evaluation, Service Plan, or Care Plan. Below, we have outlined most common use cases so you can easily identify which option suits your needs:
Create an Addendum
Create an Addendum to add new needs to a Service Plan when a resident does not currently require reassessment. These needs are typically interventions that do not impact a resident's service costs and therefore do not require new signatures.
- Example: A resident participates in your community's fall prevention program at a fixed cost as stated on their current Service Plan. Community caregivers plan to implement a new intervention as part of this program and need to add it to existing Service Plan documentation.
Edit a Care Plan
Edit a Care Plan to add a new item for documenting in Care Tracking. This standalone Care Item may be a task that is done as part of a community's routine care but not something that is necessary to capture as a service in the Evaluation and Service Plan. The added care item should also not impact service costs.
- Example: A resident requires that their hydration is monitored on a daily basis. Therefore, you add a "Hydration Tracking" item with custom outcome options so that caregivers can accurately track the resident's liquid intake in Care Tracking.
Note that you can also make changes to care task schedules when editing a Care Plan. However, it is recommended to edit the schedule on the Service Plan instead because it will ensure that both of these documents remain aligned at all times.
Edit a Service Plan
Edit a Service Plan to add or update individualized Care Item Instructions or to change the scheduling component of an existing service.
- Example: A resident has specific dressing assistance requirements, and you want to provide these instructions to caregivers. You edit the need on the Service Plan to add notes and select the "Use as Care Item Instructions" checkbox to ensure the notes are visible in Care Tracking.
Edit an Evaluation
Edit an Evaluation if an error was made when selecting an answer choice on the evaluation. This is particularly useful action if the incorrect answer choice impacts a resident's overall level of care and associated costs.
- Example: The answer choice, "Stand-by Bathing Assistance" was selected on the Evaluation in error. The resident only requires "Bathing Reminders," which is a lower level of care. Therefore, you edit the evaluation to change the answer.
Editing an Evaluation allows you to make the necessary corrections to be saved for the next assessment cycle. When you make a correction to the Evaluation, you will be further prompted to review and adjust the Service Plan.
Start a New Evaluation
Start a new evaluation if the resident is due for an assessment or if there is enough evidence to suggest that a resident's condition has changed and may require additional services than are currently being provided.
- Example: A resident's Annual evaluation is set to expire in a couple of weeks. You start an evaluation prior to that due date to remain in compliance.
Before Creating an Addendum
Before you are able to create an Addendum, you must pre-identify and configure needs in your Resident Evaluation tool. Once needs are configured, you will be able to choose these items from a dropdown menu to attach them to the Addendum. For assistance with configuring Service Plan Addendums, contact your Account Manager or our ALIS Customer Success Team.
There can only be one distinct need selected per Service Plan document, and therefore you will only be able to select a need for the Addendum if that need has not already been captured.
- Example: If you already selected "Behavior Management - Redirection" as a need on the Service Plan, you would not add this as an Addendum item because it is already included in the resident's list of services provided. However, you may want to add a distinct need, such as "Behavior Management - Safety Checks" as an Addendum item because it serves as an additional intervention for your team to action as part of the resident's behavioral management plan.
Additionally, it is not recommended that you select a need to attach to the Addendum if a related service is not already documented on the Service Plan.
- Example: If the need for Behavior Management is not already indicated on the Evaluation and Service Plan, it would not be appropriate to add a Behavior Management Intervention as an Addendum item because it is not already included in services provided to the resident. If you are trying to add an Addendum item that is unrelated to services the resident currently receives, this may indicate that a reevaluation is necessary.
How to Create an Addendum
There are multiple locations in which you can create an Addendum to a resident's Service Plan. See below for how to create access Addendums from each location:
Resident Profile - Pencil Icon
- Scroll down to Evaluations & Service Plans. In the Service Plans section, click the top righthand corner pencil icon.
- In the pop-up that appears, select Add Addendum Needs to Service Plan. Then, click Edit Service Plan.
- This brings you to the Service Plan Addendum page. Click the +Add Need button on the top righthand side of the page.
- In the side panel under the Need dropdown menu, select the need that you want to attach to the Addendum. Remember that you will only be able to choose from items that have been pre-configured for Addendums and/or are not already included in the Service Plan.
- Click Save Need to confirm your selection.
- This need now appears on the Service Plan with the "Addendum" tag. You may also see this need highlighted in red indicating that further action is required based on how it is configured in your default settings. For example, you may be prompted to set the schedule for this intervention.
- Once you have set the schedule and made any necessary changes, scroll to the top of the page and click Save Addendum.
- Both blue buttons shown on the pop-up will save and complete your Addendum. The Save & Finish button will take you to the Evaluation & Service Plan Details page. The Save & Review Care Plan button, you take you to the Care Plan section of the resident's profile.
Resident Profile - Manage Details
- In the Evaluations & Service Plans section of the Resident Profile, click Manage Details to view Evaluation & Service Plans Details page.
- In the Service Plan section, click the +Create Addendum button. This takes you to the Service Plan Addendum page in which you can follow the above listed process to add needs.
Evaluation Center
To create a Service Plan Addendum from the Evaluation Center:
- In the Evaluation Center, identify any resident's completed Service Plan. It is most efficient to navigate to the All Residents tab.
- Select the Manage Details button from this location. This takes you to the Evaluation & Service Plan Details page where you can click +Create Addendum and proceed with setup.
Edit or Delete Addendum Items
Only users with access to managing addendums will be able to use the Edit and Delete functions. As you add, edit, or delete Addendum items, ALIS will track each distinct version for you to view.
Edit
- Navigate to a completed Service Plan either on the Resident Profile or in the Evaluation Center.

You have two main options for how to start editing the Addendum:

- Click the Manage Details button, which will take you to the Evaluation & Service Plan Details page. Once there, you can click the Edit button in the Service Plan section. OR,
- Click the pencil icon, which will prompt you to select either "Make Changes to Service Plan" or "Add Addendum Needs to Service Plan." Note that either of these choices will allow you to edit the Addendum.

Delete
- Click the Delete button next to an "Additional Need," or Addendum item, in order to remove it. You will be further prompted with a pop-up to confirm deletion.
Complete
Once you have made any necessary adjustments on the Service Plan Addendum, click Complete. Then, click Complete Review on the Review Care Level page to finalize the Addendum changes.
e-Signatures
When you click Complete Review to finalize the Addendum, ALIS automatically selects the Retain Signatures checkbox. If you keep this checkbox selected when you go to click "Commit & Sign" or "Commit & Review Care Plan," ALIS will retain any signatures that have already been captured on the Evaluation and Service Plan.
The ability to retain signatures applies to both the out-of-the-box e-signature workflow and the HelloSign feature that can be enabled as part of your ALIS contract. Addendum Documents
Service Plan Addendum documents will reflect a detailed timeline of changes to interventions since the Service Plan was originally published.
You can easily view the Addendum .pdf document in the Evaluations & Service Plans section of the Resident Profile. When you click on the paper icon, you can select which documents to download. Make your selections and click View to download the .pdf.
You can also view the .pdf by navigating to the Evaluation & Service Plan Details page. When you click View, the .pdf document will automatically download for you to review, save, or print.
The Addendum .pdf shows a timeline of services added, edited, and ended.
