By default, the Health Profile section appears in the Evaluation workflow and includes key resident demographic and clinical information. If your community does not require this information to appear on Evaluation and Service Plan documents, administrators can disable it using a system setting. This setting hides the section from both the evaluation workflow and all generated documents.
Health Profile Section in Evaluations
The Health Profile section appears at the top of the evaluation and includes key resident demographic and clinical information.
- This section is a direct mirror of the Resident Profile
- Updates made in the evaluation automatically sync to the Resident Profile (and vice versa)
- Changes in this section require clicking Update to save (no auto-save)
Note: In addition to the Health Profile, ALIS-synced sections also include Immunizations and Tests and Contacts.
Health Profile on Evaluation and Service Plan PDFs
When the setting is enabled, the Health Profile populates the cover page of both Evaluation and Service Plan PDFs to ensures that core resident information is included in documents used for review, sharing, and signatures.

When the Setting Is Enabled vs. Disabled
The visibility of the Health Profile is controlled by a community-level Care Setting.
Enabled (default): Displays the Health Profile as the first page of the evaluation and includes it on all generated Evaluation and Service Plan PDFs.
Disabled: Removes the entire Health Profile cover page from both the interactive evaluation experience and all exported PDFs.
⚠️Important:
- The setting applies to the full section only; individual fields cannot be hidden
- The Contacts section is not affected and has its own independent toggle
- Signed documents are not updated and will reflect the Health Profile as it appeared at the time of signing
How to Configure the Setting
Navigate to Settings → Care → General tab.

Under Care Planning Options, locate and adjust the “Display Health Profile on Evaluations and Service Plans” toggle.
Move to Enabled (default) to display the Health Profile
Move to Disabled to remove it from evaluations and PDFs
The setting will save automatically.
