Evaluations and Service Plans can be edited if you find a detail that needs to be changed or updated and does not require an entirely new Evaluation or Service Plan to be created.
It may be that the detail was a mistake to begin with, or that you need to adjust points or fees associated with the assessment.
Editing Evaluations
To begin editing an evaluation, navigate to the Evaluation Center or the Resident Profile.
Important: When editing the Evaluation, previous answer choices are saved. However, when you edit specific answer choices in the Evaluation, those reset in the Service Plan and require rescheduling. You can retain collected signatures on the Evaluation.
via the Evaluation Center
- Go to the Evaluation Center.
- Locate the evaluation in the In Progress, Needs Signatures, or All Residents tabs.
- Under Evaluations, click the edit pencil icon.
- In the pop-up, click Edit Evaluation to confirm and begin making changes.
via the Resident Profile
- Go to the Resident Profile.
- Locate the evaluation in the Evaluations & Service Plans section.
- Under Evaluations, click the edit pencil icon.
- In the pop-up, click Edit Evaluation to confirm and begin making changes.
- On the Evaluation, make any edits and click Complete when finished.
- Review the Next Evaluation Reason and Next Evaluation Date.
- Be sure to keep the Retain signatures checkbox selected.
- Click Go to Service Plan. This takes you to the Service Plan page where you will be prompted to make any necessary scheduling updates.
Editing Service Plans
Service Plans can be edited via the Evaluation Center or Resident Profile.
- Under Service Plans, click the edit pencil icon.
- You will be prompted to either Make Changes to Service Plan or Add Addendum Needs to Service Plan.
- To simply edit the service plan, select Make Changes to Service Plan.
- Click Edit Service Plan.
- On the Review Service Plan screen, make any desired edits to the Service Plan.
- Click Complete Service Plan when finished. This takes you to the Review Care Level screen for you to verify Care Levels and Fees.
Important: After completing edits to the Service Plan, re-publish the Care Plan on the resident profile so that Care Tracking stays in sync with your changes.
Editing Care Levels and Fees
Editing the Level of Care is considered part of the Service Plan. It is possible that the fee needs to be adjusted due to a recent rate change. The ability to edit Care Levels and Fees requires specific security role permissions.
- Locate the Service Plan either in the Evaluation Center or the Resident Profile. Click Manage Details.
- On the Evaluation & Service Plan Details page, click the edit pencil icon on Care Level or Fee.
- Click Edit Service Plan.
- On the Review Service Plan page, click Complete Service Plan.
- On the Review Care Level page, click Override Care Level.
- Enter the updated Level and/or Fee, and click Override Care Level to save.
- Click Complete Review.
- Be sure to keep the Retain signatures checkbox selected.
Important: After completing edits to the Service Plan, re-publish the Care Plan on the resident profile. Ensure you commmunicate fees changes to your Billing team.
Adding an Addendum to a Service Plan
To add an addendum to a Service Plan, select the Add Addendum to Service Plan option and click Edit Service Plan. Review the Service Plan Addendums article for a more in-depth explanation of addendums.
If the addendum option is not available to you when editing a Service Plan, it may be due to your security role access or the feature has not yet been enabled for your company. Contact your Account Manager for assistance.
Retaining Signatures
If you edit one of the documents, you have the option to retain previously recorded electronic signatures. Upon completion of an edited Evaluation or Service Plan, make sure the "Retain Signatures" checkbox remains checked.
