The Evaluation Cycle is the standardized, three-part workflow used in ALIS to assess a resident's health, determine their specific needs, and build a clear plan for their daily care.
When you begin an evaluation, you are initiating a connected process that moves through three distinct stages:
1. The Evaluation
This is the starting point. The Resident Evaluation Tool (RET) consists of a customized list of questions designed to comprehensively assess a resident's health and acuity.
Purpose: To gather the clinical data needed to determine the resident's Level of Care (LOC) and required services.
2. The Service Plan
Once the evaluation is complete, ALIS generates the Service Plan. This document translates the evaluation answers into a list of specific needs.
Purpose: This serves as the contractual agreement between the community and the resident (or responsible party). It is also where the clinical team schedules the specific services—such as medication management or bathing assistance—that the resident requires.
3. The Care Plan
The final output of the cycle is the Care Plan. This is the actionable schedule that flows directly into Care Tracking for your staff to document against every day.
Purpose: To provide caregivers with a clear, time-specific list of tasks based on the services scheduled in the Service Plan.
Note: While the specific questions in your Evaluation are customized to your organization, the steps to complete this cycle remain consistent. All three components can be generated as PDFs for physical signing or printing.
