The state of Florida has a regulation, requiring communities to have an up to date admission and discharge log, with specific information.
The report in ALIS is based on the following Florida state regulation:

58A-5.024 (b) An up-to-date admission and discharge log listing the names of all residents and each resident’s:

Date of admission, the facility or place from which the resident was admitted, and if applicable, a notation indicating that the resident was admitted with a stage 2 pressure sore; and,

Date of discharge, reason for discharge, and identification of the facility or home address to which the resident was discharged. Readmission of a resident to the facility after discharge requires a new entry in the log. Discharge of a resident is not required if the facility is holding a bed for a resident who is out of the facility but intending to return pursuant to rule

58A-5.025, F.A.C. If the resident dies while in the care of the facility, the log must indicate the date of death.
In ALIS, this is an ALIS HQ report called “Admission/Discharge Log”.
The report in ALIS matches the state requirements nearly exactly, though there’s a couple of things to note:

If a resident has a stage 2 pressure sore, make sure to enter “stage 2 pressure” or “stage 2 wound” in the primary or secondary diagnosis field of the health profile. The report will pull this info into the report.

The "discharge date" column of the report will pull in whatever date is entered in the physical move out date. (If you need to adjust this to a be a different date of death, export the report to excel and you can enter any changes.)


Column meanings:

Admission date = move in date

Type = will always say “new admit”

Admitted from = what is selected from the “moving in from” dropdown in the resident profile “move in and community info” blue bar

Admitted from address = what is entered in the “moving in from address” field in the resident profile “move in and community info” blue bar

Stage 2? = will say “yes” if “stage 2 pressure” or "stage 2 wound" is entered into the primary or secondary diagnoses field of the resident profile “health profile” blue bar

Discharge date = physical move out date (this is the implied date of death, if the discharge reason is selected as “expired” or “death”. You can adjust this discharge date if needed, by exporting the report.)

Discharge reason = what is selected in the “move out reason” dropdown in the “move out information” resident profile blue bar

Placement after discharge = what is entered in the “move out destination” field in the resident profile “move out information” blue bar

Placement Address = what is entered in the “move out destination address” field in the resident profile “move out information” blue bar
