The Evaluation Center helps you keep track of all your residents' and applicants' Evaluation and Service Plan statuses. You'll be able to tell at a glance whether some residents are due for a clinician to review their needs.
In this guide we discuss all details of the Evaluation Center, as well as the Evaluation & Service Plan Details page.

- The Evaluation and Service Plan Details page is showing you information about a specific evaluation record for a specific resident. The information on this page determines what will display in the Evaluation Center.
Evaluation Center
To get to the Evaluation Center, click the icon in the Care section of your ALIS home screen, or find it in the Residents menu at the top of ALIS.
Here we have three tabs that organize evaluation completion tasks:
- Needs Attention
- In Progress
- Needs Signatures
The All Residents tab shows us the most recent evaluations records for everyone.
The Print Blank Evaluation button generates for you a blank template of your custom evaluation in ALIS. This may be useful if you must use pen and paper to track the answers to questions on the go.
The Start New Evaluation button allows us to begin or re-complete the process for any resident who does not have a currently In Progress Evaluation or Service Plan.
Needs Attention
Start in this section to see which residents' evaluations require your attention!
- This section first displays the list of all Applicants in your building that do not have an Evaluation and Service Plan on file. Since it is very important to have a clinical review of individuals who are new to the community, this always displays first.
- Next, we show any Expired Evaluations. Expiration dates are determined at the end of the most recent Evaluation on file, and can be updated on the Evaluation & Service Plan Details page. It is a good practice for Evaluations to be reviewed every three or six months for residents based on their acuity.
- After the Expired section, the rest of the page gives a calendar view for many months in the future, each month showing you the number of re-evaluations due. If you notice that many are due clustered around one month or a few months, you can choose to complete some re-evaluations early in order to give yourself more time to complete them before they expire.
Note: Any re-evaluation after the first one will contain all the same responses from the most recent evaluation when you get started, so you only need to review the details and update anything that has changed for the resident!
In Progress
Here we have any Evaluation or Service Plan that has been started but not Completed. Based on the document that is incomplete, you can click the Continue button to pick up where you left off.
It is advised to prioritize completing any Evaluation or Service Plan that is in this status. If a document sits in this status for a long time, it is possible that you may need to update the dates on the documents, review all details again to make sure they are still accurate, and essentially start fresh with the whole process.
Evaluations expire on a regular basis, so if it is time to re-evaluate and you discover the previous evaluation was never finished, it is less obvious how to save both the previous and the current evaluation as separate records.
Note: The Manage Details button in the Evaluations column takes you to the Evaluation and Service Plan Details page.
Need Signatures
This tab includes all residents whose most recently completed Evaluation and/or Service Plan is lacking at least one required signature.
The role of each signer and number of signers on the Evaluations and Service Plans are set in Resident Evaluation Tool Settings, and these can be customized for your organization's needs.
If one or more of a resident's documents has outstanding required signatures, there will display a Sign button that takes us to the Evaluation and Service Plan Details page. The Manage Details button goes here too.
All Residents
This tab shows us the most recent Evaluation and Service Plan records for every Applicant and Current Resident.
- Use the filters to find a specific resident, look over the people within a single Product Type, or use Filter By to view the evaluations in a specific stage of completion.
- By default the residents are sorted alphabetically by last name, and you can update the Sort By filter to show you the list in order of room number.
- The Export to Excel options give you this information in greater detail for all the most recent documents on file if you select Export Current to Excel. If you Export Historical to Excel, you can see more details about previous documents too. These Excel files contain a lot of information that you might not find in other reports!
If a resident has no evaluations on file, there displays two buttons: Start New Evaluation or Add Historical Evaluation.
The Add Historical Evaluation option allows you to import the date, expiration date, and Care Level of an evaluation completed outside of ALIS. This will help you keep track of when you need to complete the next evaluation, and it defers the completion of an ALIS Evaluation, Service Plan, and Care Plan to a later date.
Note: This option does not help you reach the goals of completing Service Plans for all residents, or going live with Care Tracking based on the Care Plans.
We can also import these dates and care levels if needed. For help importing, contact your onboarding manager or ALIS Support!
Evaluation and Service Plan Details
This page helps us review all the required dates, documents, and signatures for a resident's most recently completed Evaluation and Service Plan.
To get here, click Manage Details or Sign on any resident Evaluation or Service Plan.
- The top section shows us some resident basic details, and below that is where we see the key details of the evaluation, including Care Level and Fee if the Service Plan has been completed. These sections are editable, but you will be prompted to publish a new version of the Service Plan if you want to edit these details.
- If you have completed the Service Plan, you may have an orange alert and button prompting you to Review Care Plan for this resident. This button takes you to the Care Plan section of the resident's profile.
- The main sections and most utilized parts of the page separate the Evaluation on the left and the Service Plan on the right. This is where we collect signatures.
- On the left in the Evaluation section, we can edit the expiration date and reason for the next evaluation.
In both the Evaluation and Service Plan sections, click the Sign link for all signatures you can collect electronically using ALIS. Typically these include any of your colleagues at the building, such as a Nurse, Wellness Director, or Executive Director.
Once you have all internal signatures collected, you may need to collect a signature from the resident, their family, or an outside party. These can be done electronically if possible, but you may need to print the document so that they can sign it on paper.
You can upload the signed paper copy of the document to this page again using the Upload button. This is a good way to keep track of the signed document in ALIS so you can easily find it later. If you choose to do this, make sure you scan the entire document and upload the file in its entirety. The item you upload will fully replace the document if you need to access it later.
